The church was built in 1388 on the spot where Mary appeared to Pelingo, a nobleman of Acqualagna. In 1422 a woman from the same town commissioned an image of Mary which was placed in the chapel of the church. The sacred image survived the disastrous earthquake of 1781 and became highly venerated. Shortly afterwards, another local man prayed to the image to spare his ill daughter. According to tradition, his prayers were answered and the miraculous healing led to the construction of a new chapel and a deeper devotion to the image. The shrine is managed by the sisters of the Sacred Heart while the newly renovated guest house is operated by the Archdiocese of Urbino.
